@Cynthia - Federally Qualified Health Center = FQHC
can you list the tools that you are talking about on a slide??
I have a good story I could share about having a convrstion that helped at end of life
Thanks so much Dr. Thomas, I'll call on you.

@All - Which of these outcomes on this slide do you already measure? Could any part of that measure be used to assess age friendliness?
so sorry, I am back
No problem, we've learned to be flexible with ZOOM!
@All - Where would What Matters documentation best fit within your own EHR?
Enjoyed the conversation - I have to drop off for another CMS webinar.
Great Story! Thank you
Some take home thoughts I gained from today's event.... "What matters" is not a question, but a conversation. Dr. Elliot's comment about the 4Ms focus on strength and wellness of a person/patient.
@All - What are the signs that clue YOU in to the need to further evaluate your patient’s Mentation?

@All - Which of the outcomes on the slide do you already measure in your setting? Could any part of that measure be used to assess age friendliness?
What are some tips to get people interested and committed to this when staff have so many competing priorities?
We have been running into quite a few older adults (many happen to be male) who stubbornly refuse home/community based services even though they would benefit them maintaining their home experience more successfully. Any advice on creative approaches?
Have you folks seen efforts in your jurisdictions to get hospital staff trained in conducting tools like the Mini-COG, MOCA, etc.? In Hawaii, Office of Elderly Affairs is looking at having social workers trained in conducting these tools telephonically to help with early identification and diagnosis.
For those of us in LTC/SNF, the MDS uses the PHQ-9 and BIIMS and CPS story data is available
Some time and effort on the front end saves a lot of time in the future Trying to get staff to learn this will really help them in the next days and weeks
Alternatives utilizes the MMSE.
We can also talk more about these questions and shared tools in the Office Hours as well. Please join us for the discussion.

regarding patient's primary caregiver, specially patients with cognitive /mentation changes, they are good source of information. after evaluating a patient and getting as much information from her , i also got needed information from the husband , her primary caregiver. When i'm done with my assessment, i asked him " how are you?'" He started to repeat again what he's been dping for the patient. I respoded "i don't think you understand my question." let me repeat myself." " how are you ?" He looked at me and said ' You're the only one who asked me that ." Let's not forget our patient's caregiver too.
@Joshua - thank you for sharing this story!
